TRR´s (continuação)


É importante ressaltar que o pensamento multivariado rejeita rótulos de bons e ruins absolutos, e estamos aqui justamente para tirar o melhor dos TRR´s.

Até mesmo pessoas como Rinaldo Bellomo, sempre citado para amparar TRR´s, já estão pensando diferente em relação a tema. Em recente artigo intitulado Clinical deterioration in hospital inpatients: the need for another paradigm shift? MJA 196 (2) · 6 February 2012, Bellomo e colaboradores escrevem:
The MET [medical emergency team] model is reactive, as it requires a patient to deteriorate before it can be activated. We contend that a new paradigm is needed to promote a more proactive approach to recognising deterioration, and to provide a pre-emptive, structured, standardised approach to managing patients who are known to be at risk of deterioration. 
E então, como sugestão principal, sugerem abordagem prioritário dos pacientes antes da aparição dos warning signs, através de:
  • Advance care planning
  • Medical comanagement of some surgical patients 
Opa, estamos falando de boa assistência regular...

Evitar

Com Michael DeVita, no NEJM, Bellomo escreveu também que:
Potential adverse effects of the implementation of a rapid-response system include additional cost, diversion of resources that could be used to care for critically ill patients, and a decreased sense of responsibility for patients on the part of the hospital-ward team.
Although rapid-response systems are assumed to be models for advancing patient safety, they should always be part of a much wider strategy aimed at making modern hospitals safer.
O modelo utilizado por Bellomo e DeVita foi multivariado (certamente eles não são contra TRR´s agora), ou seja, consideraram todas as variáveis que se associam de forma independente com o benefício e o risco. O mais difícil desta abordagem é lidar com a ausência de recomendações finais tipo sim ou não. Conclui-se com vários “se´s“ e “porém´s“. E, para quem só aceita a lógica univariada, soa como ausência de conclusão.
